The man, John, is 55 years old, weighs 80 kg, and is 167 cm tall. He is married with two children and lives with his wife. He works as an accountant and does not have any major stress at work or home. He is a non-smoker and drink alcohol sometimes. His past medical history includes hypertension, hyperlipidemia, and type 2 diabetes. His medications include: - Amlodipine 5 mg daily. - Atorvastatin 20 mg daily. - Metcormin 1000 mg twice daily. Mr. John entered the emergency department complaining of chest pain and shortness of breath during the past two hours. He describes the pain as sharp from the left side of his chest and extending to his left arm. He describes the intensity of the pain as 8/10, accompanied by sweating and nausea. He denies the occurrence of any palpitations, dizziness, or Fainting. his father died of myocardial infarction at the age of 60, and his mother had a history of high blood pressure and stroke. After performing the physical examination:
-Vital Signs: BP 160/90 mmHg, HR 100 bpm, RR 20/min, Temp 98.6\deg F, O2 saturation 95% on room air.
- General: Appears uncomfortable, diaphoretic.
- Cardiovascular: Regular rhythm,no murmurs. Respiratory: Clear breath sounds bilaterally.
- Abdominal: Soft, non-tender, non-distended. Diagnostic tests:
-ECG: Sinus tachycardia, ST
-segment elevation in leads II, III, aVF, V5, V6.
-Troponin: Elevated at 0.8 ng/mL (normal range < 0.04 ng/mL).He was found to have acute lower ST-elevation myocardial infarction (STEMI).
