Assignment four is where you have the opportunity to tell us the
detailed story of the events that led you
to this program. We are
interested in learning about your drug experimentation and the
consequences of your using. Write your 500-word story on a
separate piece of paper. You may handwrite this assignment as long
as your writing is legible. If you choose to type the essay, it must be
12-point font, 500 words, with 1 inch margins, and you must provide
us with a printed copy of your story.
Your Story must include the following:
1. When and why did you start using?
2. How old were you when you first started using substances?
3. What substances have you tried, and what is your preferred drug?
4. Who first introduced you to drugs (i.e., friends, relatives, strangers)?
5. If you continue your use, where will you most likely end up: hospital,
prison, or mortuary?
6. What is your definition of an addict?
7. Would you consider yourself an addict? Why or why not?
8. What goals can you set for yourself to change your path?
9. End your Story explaining how you feel about this statement:
"Everyone in the JADE program is at the fun with problems stage of
addiction; if someone continues to use drugs, they have a problem
despite negative consequences."
