Procedure Report PREOPERATIVE DIAGNOSIS: Unstable progressive angina pectoris, markedly abnormal thallium exercise test, 95 percent circumflex coronary artery stenosis

PROCEDURE: PTCA of left circumflex with stent placement

PROCEDURE DETAILS: Following 1 percent Xylocaine local anesthesia in the left femoral region using the Seldinger technique, 8 French sheaths were inserted in the left femoral artery and vein. A 7 French Swan-Ganz catheter was advanced in antegrade fashion through the right heart chambers and positioned in the pulmonary artery. Selective left coronary angiography was performed with an 8 French JL4 curved short tipped guiding catheter, after which time a 0.014 in microglide exchange wire was passed down the length of the circumflex coronary artery, and a 2.5 mm Shadow balloon dilatation catheter was advanced over the wire and inflated to a maximum of 9 atmospheres on multiple occasions. This balloon catheter was then removed, and a 2.5 mm Cook stent was placed over a 3.0 mm. compliant balloon catheter in the circumflex main trunk. This balloon was inflated to 9 atmospheres and kept inflated for 4.0 minutes. The balloon catheter was then removed. Selective angiography was performed, which demonstrated excellent flow and run-off in the circumflex and no evidence of occlusive type dissection. At this time, the arterial and venous sheaths were sutured in place. The patient was taken to the holding area of the catheterization laboratory without incident. There were no complications resulting from the procedure.
