Post-operative Observations and Indications
Vital Signs: Check blood pressure, heart rate, respiratory rate, and temperature for hemodynamic stability and early signs of infection or shock.
Pain levels should be assessed on a regular basis using a pain scale to manage discomfort and identify potential complications.
To identify infection or bleeding, inspect the wound and dressing for redness, swelling, or excessive drainage.
Drain Output: Record the amount, colour, and consistency of drainage to check for internal bleeding or infection.
Abdominal Assessment: Listen for bowel sounds and abdominal distension to evaluate gastrointestinal function and detect complications such as ileus.
Urine Output: Check for adequate output to determine renal function and hydration levels.
