Predictive analytics was used in three hospitals with the goal to decrease readmission rates. In the first case UnityPoint Health added an open-ended question for patients and tied it to data, creating a readmission risk score for patients that can be shared with their provider/care teams. The tool can then be used to help identity onset of symptoms and dodge a readmission. After 18 months of using the predictive analytics tool UnityPoint Health reduced admissions by 40%. The benefits absolutely support the tool, 40% is a huge shift in reducing readmissions. (Cryts, 2019)The only risk I can identify is the possibility of incorrect risk scores that may then cause someone to not be seen whether that is in the ED or primary care. Without capturing or reporting out data to see how often this occurred in the 18 months or moving forward leaves the risk low.
