A 58 year old lawyer came to the emergency room for the clinic in agonizing pain. He states about one week ago his jaw neck and stomach felt tight and he had difficulty swallowing. Currently he has severe muscle spasms in his left arm, chest and back and cant exhale. He states that he injured himself about 10 days earlier, puncturing his left arm with a nail from an old barn. He is tearing down. The wound has produced moderate quantities of pus, but he has been keeping it clean. The patient was admitted to the clinic. The wound was sampled for microscopic examination and culture. Gram-positive bacilli were observed in a Gram stain and endospores were observed in an endospore stain of the pus. The culture only grew in anaerobic conditions. The patient was given antitoxin injections prescribed antibiotics, narcotics, and muscle relaxers, and placed on a ventilator. The patient underwent hyperbaric oxygen therapy in the clinic.
