Lung sounds clear bilaterally to auscultation. Abdomen with tenderness and hyperactive bowel sounds. States, "I'm so thirsty all the time." Urinating small amounts of dark amber urine. States that has sore on ankle that won't heal. 0900 Client reports abdominal cramping with diarrhea. 1100 Client states, "I feel so weak."
