This patient is having anemia, acidosis, hyperkalemia, hyperglycemia and renal insufficiency. The PR depression on EKG and the pericardial friction rub are signs of pericarditis. An echocardiogram may reveal additional useful information about the extent of pericarditis and the presence of an effusion. The patient’s poor performance on the mini mental status exam may be an indication of encephalopathy.
